Hard lump on the inside of his knee

DEAR ASK THE DOCTOR: My 37 year old brother has a relatively hard lump on the inside of his knee.  It almost looks like the condyle on his distal femur is enlarged.  However, the lumb is present when his knee is extended and seems to "disappear" when he flexes his knee.  He says it is not painful at all.  Any ideas on what this could be?
